Camp Paperwork

Please be sure to have the following forms completed and  returned as soon as possible. Having these forms completed prior to your arrival will speed up the check-in process. You may scan, fax or mail these forms to the CCO office if you'd like.
          

Parent Release & Medical Exam Form:
Your campers health and safety are our first priority.  Having health forms  prior to your camper’s arrival for thorough review by our health care staff is critical.


Behavior Contract:
This form is for all campers age 13+ who are participating in our Challenger, Sherpa, Trekker, and Leadership Programs.  It establishes expectations of conduct during our special teen programs.


Rafting Waiver:
Please fill out this form ONLY if you are participating in the Traditional Camp rafting day trip.
